General information:

C1 esterase inhibitor is a protein that is produced mainly in the liver and is found in the liquid part of the blood.
A C1 esterase inhibitor controls C1, which is the first component of the complement system.
The complement system is a group of proteins that work with the immune system and play a role in the development of inflammation.

This test is useful when a person has signs of hereditary angioedema (C1 esterase inhibitor deficiency).
C1 esterase inhibitor deficiency causes swelling of the face and throat. Bowel swelling can cause severe abdominal pain that can be confused with acute abdominal conditions such as appendicitis. Skin swellings are deeper and larger than urticaria ("hives"). They can be painful and not itchy.

Symptoms usually begin in adolescence, but can occur at any age. Sometimes C1 esterase inhibitor deficiency is asymptomatic.
